What do we examine during the mandatory ultrasounds?

The first trimester ultrasound can be performed between 11 weeks and 4 days and 13 weeks and 6 days, with the optimal time being between the 12th and 13th weeks. It is important for the expectant mother to arrive with a full bladder, as this significantly improves the visibility. The examination is usually abdominal, but a vaginal ultrasound may also be required if necessary.
What happens during this examination?
Determining the exact age of the pregnancy and the estimated due date: This ultrasound is the best time to accurately estimate the baby's age, as individual developmental variations may occur later on.
Detailed anatomical examination of the baby: checking body parts and organs.
Early examination of the heart and central nervous system: Although this is not part of the standard protocol, we consider it important to perform this examination at this stage as well.
In combined screening, precise measurement of ultrasound markers used for risk assessment.

Second trimester ultrasound
The second trimester ultrasound is the most detailed anatomical examination during pregnancy. It can be performed between the 18th and 22nd weeks, but it is best conducted between the 19th and 20th weeks, so it's advisable to schedule the appointment accordingly. For this examination, it is preferable to arrive with an empty bladder, and a vaginal ultrasound may also be performed to check the length of the cervix and assess the risk of preterm labor.
What do we examine during this ultrasound?
The baby's anatomy.
We thoroughly check the heart and nervous system, as well as the body's anatomy.
We measure the cervical length to assess the risk of preterm labor.
We check the position of the placenta.
What we examine, even though it is not part of the protocol:
Development of certain elements of the central nervous system: the presence of major brain structures, the formation of brain sulci, etc.
We perform an extended screening of the heart, which can detect more than 90% of severe congenital heart defects, whereas the standard protocol examination identifies only 60% of these conditions.
We screen for placental implantation abnormalities – the risk of these issues increases after any uterine surgery, and it is crucial to detect them in a timely manner.
Third trimester ultrasound
This ultrasound is performed between 30 and 32 weeks and aims to provide a general assessment as well as to examine any potential abnormalities that may not have been detectable earlier. Arriving with an empty bladder is ideal, and a vaginal ultrasound may be considered for a more detailed examination of the central nervous system (if the baby is in a head-down position).
Certain abnormalities of the major vessels: smaller but potentially significant stenoses may only become visible during this stage of pregnancy.
Screening for certain abnormalities in the kidneys, intestines, and skeletal system: These organ systems provide a much more detailed picture in the third trimester, making it important to revisit them. For instance, potential intestinal dilations, though rare, could suggest a congenital stenosis. This allows for advanced planning, ensuring that increased attention can be given immediately after birth if needed.
Re-evaluation of the central nervous system: By this stage, the brain has developed significantly, making it crucial to re-examine it to assess any further developments or issues.
Monitoring fetal growth rate
Checking the amount of amniotic fluid: both too little and too much can indicate potential problems.
Checking placental function – it can signal potential growth delays weeks in advance!
Final check of placental position: this can provide important information regarding the delivery method.

Term-near condition screening
This examination used to be mandatory, but unfortunately, it has been removed from the protocol. However, we consider it important and strongly recommend having it done between the 36th and 38th weeks.
What do we examine in this condition screening?
Checking placental function: we assess whether the placenta is still able to meet the baby's needs.
Weight estimation: we estimate the expected birth weight of the fetus.
Fetal position: we check how the baby is positioned, as this is important information related to the delivery method, and it can still change after the 30th-32nd week.
Genetic ultrasound at L33, where there is time to examine everything thoroughly!
The list of organs to be examined is already long, and if we want to check everything thoroughly, it cannot be completed in just 5-10 minutes.
As we mentioned earlier, we perform measurements and checks that are not part of the standard ultrasound protocol but are important to us. However, this requires additional time.
Sometimes the baby may not be in an ideal position or may not be very cooperative, making it more difficult to see what we need to.
We consider it important for the future parents to see and understand what we are observing and discussing. Therefore, we show them everything, explain it thoroughly, and answer any questions they may have.
We don't only focus on the pregnancy itself: the well-being and medical history of the expectant mother are also important, as these factors may indicate the need for additional examinations.
Everyone can always have one more question: it is important to us that no one leaves the office with doubts or questions. We never rush so much that there isn’t time to ask one more question.
For all these reasons, we work without time limits!
It's advisable to allocate 45-60 minutes for an examination. We have found that this amount of time is necessary to thoroughly and meticulously check every detail of a baby in the womb without rushing, while also involving the parents.
However, this time frame is only a guideline: the duration of the examination depends on factors such as the baby's position, cooperation, movement, the visibility conditions, and the thickness of the mother's abdominal wall. Therefore, we recommend that the expectant parents avoid scheduling any urgent appointments on the day of the examination.
