Thyroid

One of the most common endocrine disorders is thyroid disease, which is considered widespread in our country. Hormones produced by the thyroid gland play a role in metabolic processes, brain function, growth, and sexual functions. In fact, there is hardly any organ whose function is not influenced in some way by these hormones.

Thyroid diseases:

Hypothyroidism

Symptoms of hypothyroidism include drowsiness, fatigue, hair loss, dry skin, weight gain, constipation, decreased libido, menstrual disorders, cold intolerance, and depression.

Hyperthyroidism

Symptoms of hyperthyroidism include rapid heartbeat, irritability, diarrhea, excessive sweating, unexplained weight loss, tremors, insomnia, and menstrual disorders.

Inflammatory diseases

Inflammatory diseases can be autoimmune, viral, or bacterial in origin, with bacterial cases being very rare and potentially severe. They can also occur postpartum, typically in a transient form. Symptoms vary and can often be challenging to identify, often presenting as a hormonal dysfunction. In cases of true inflammation, symptoms such as upper respiratory tract infections and fever precede the condition, with redness, sensitivity, and swelling in the neck area.

Thyroid enlargement – goiter

Goiter can be diffusely enlarged or nodular, often without hormonal dysfunction. Signs may include neck thickening, palpable or visible nodules, breathing difficulties, swallowing issues, and hoarseness.

Tumors

Thyroid tumors are mostly detected in an asymptomatic state through ultrasound examinations followed by cytological tests.

In the case of suspected thyroid disease, detailed investigation is crucial to confirm the nature of the condition and, if possible, its cause, followed by appropriate treatment.

Hyperprolactinemia
  • Increased hormone secretion by the pituitary gland, resulting in excessive production of the hormone that stimulates milk production.
  • It occurs physiologically during pregnancy and breastfeeding. Other causes need to be investigated and treated.
  • The most common causes are a small, benign tumor of the pituitary gland, hypothyroidism, medications, and PCOS (polycystic ovary syndrome).
  • It causes menstrual disorders, infertility, milk discharge from the breasts, and decreased libido.
PCOS

The so-called polycystic ovary syndrome (PCOS) affects approximately one in ten women and is currently the most common cause of infertility.

The name comes from the fact that, during an ultrasound examination, many small cysts are visible in the ovaries beneath the cortex.

In this condition, there is a hormonal imbalance in the ovaries, characterized by an excess of male hormones. This can lead to irregular bleeding, increased hair growth, hair loss, acne-prone skin, and excessive sweating. Ovulation is often absent, as cysts form from unripe follicles beneath the cortex, leading to infertility. Weight gain is also common.

In the case of suspected PCOS, it is crucial to clarify the underlying causes, which requires a detailed hormonal evaluation. Additionally, it is advisable to conduct tests for insulin resistance as part of the assessment. This involves a glucose tolerance test, where both blood glucose levels and insulin levels are measured.

In insulin resistance, the organs involved in carbohydrate metabolism become insensitive to insulin, primarily resulting in reduced glucose uptake by these cells. As a result of elevated blood glucose levels, the pancreas starts to secrete more insulin.

This also has various other metabolic effects and can lead to diabetes in the long term.

Among the symptoms, it is important to mention, in addition to hormonal effects, frequent cravings for sugar, increased desire for carbohydrates, weight gain, poor sleep, night sweats, fatigue, concentration problems, unsuccessful dieting, and symptoms occurring with missed meals: sweating, trembling, dizziness, and irritability.

These symptoms can occur even in the absence of PCOS or hormonal issues.

Hyperparathyroidism

The increased production of parathyroid hormone, known as parathormone, is the cause, and several underlying factors may be involved: renal failure, absorption disorders, or a benign tumor of the parathyroid glands. There are four parathyroid glands, located behind or beside the lobes of the thyroid gland.

Parathyroid hormone regulates the body's calcium metabolism. In cases of hyperparathyroidism, calcium levels in the blood increase, which can lead to heart rhythm problems, as well as kidney stones and osteoporosis.

Addison's disease

A condition caused by the insufficient hormone production of the adrenal cortex and the destruction of adrenal tissue, characterized by severe illness, high levels of weakness, fatigue, weight loss, low blood pressure, disturbances in electrolyte balance, metabolic collapse, and circulatory failure.

Conn's syndrome

Conn's syndrome is characterized by excessive production of adrenal cortex hormones, particularly aldosterone, usually due to a benign adrenal tumor or tissue proliferation. The typical symptoms include difficult-to-treat high blood pressure and low potassium levels in the blood.

Cushing's syndrome

Cushing's syndrome is caused by the overproduction of the hormone cortisol by the adrenal glands. This condition can result from both benign and malignant tumors, which may occur in the pituitary gland, the adrenal glands, or, though less commonly, other organs.

Cushing's syndrome leads to a wide range of symptoms due to the multifaceted effects of cortisol: abdominal and chest obesity with thin limbs, moon facies, high blood pressure, elevated blood sugar levels, electrolyte imbalances, osteoporosis, bone fractures, dry skin, thinning skin, purple striae, fatigue, susceptibility to depression, edema, reduced libido, and in women, menstrual irregularities and increased hair growth.

Phaeochromocytoma

Phaeochromocytoma are generally benign tumors that produce catecholamines (such as adrenaline, noradrenaline, and dopamine). These tumors can originate from the adrenal medulla or, less commonly, from outside the adrenal glands.

The increased production of these hormones often causes symptoms that occur in episodes: high blood pressure, headaches, rapid heartbeat, sweating, chest and abdominal pain, restlessness, and pallor.

Acromegaly

Excessive production of growth hormone in adulthood is caused by benign tumors of the pituitary gland. It is characterized by enlargement of certain bones, muscles, and internal organs. Visible changes are primarily observed in the face: larger nose, forehead, wider face, protruding chin, oily skin, and increased size of hands and feet.

Additionally, heart problems, migraines, visual disturbances, high blood pressure, diabetes, erectile dysfunction, menstrual issues, infertility, decreased libido, and obesity can develop.

Diabetes insipidus

The absence of the antidiuretic hormone (ADH) from the pituitary gland causes very characteristic symptoms: constant thirst and increased fluid intake, along with increased urine output. This can range from 3-5 liters per day to as much as 18-20 liters per day in more severe cases.
