Pregnancy confirmation – What do you need to know about the first examination?

One of the most exciting moments of expecting a baby is when the pregnancy test turns positive. Many want to see their gynecologist immediately to see their baby and hear its heartbeat. The following description is for informational purposes only. It cannot be overstated that each individual is different, every pregnancy is unique, and every expectant mother has her own specific circumstances. Therefore, like many things in life, this must be tailored to the specific situation and patient.

Registration

At 5-6 weeks (from the first day of the last menstrual period): initial visit. Detailed medical history, examination, transvaginal ultrasound, and pap smear.

Week 8-10

Follow-up transvaginal ultrasound, then as needed: enrollment in midwifery care, general practitioner and dental examination, ECG, and laboratory tests. We will discuss genetic tests with the expectant mother to determine the most suitable options for her.

Week 12

Ultrasound measurement of the nuchal fold, with the option for combined/integrated testing and NIPT testing. We discussed these earlier. Follow-up with results and blood test results.

Week 16

In the case of integrated testing, a second blood sample will be taken. Follow-up gynecological examination, during which blood pressure measurement, cervical check, and, if necessary, ultrasound examination will be performed.

Week 18-20

Genetic ultrasound examination, followed by a gynecological check with the results.

Week 24-26

An oral glucose tolerance test (OGTT) with blood sampling, during which we also check many other factors besides glucose. Follow-up with results, including the essential blood pressure measurement, and a vaginal ultrasound examination to measure the cervical length.

Week 28

Cervical check, blood pressure measurement, and urine test at the clinic.

Week 30-32

Ultrasound examination, gynecological check: cervical assessment, blood pressure, and urine test.

Week 33

If the previous examination was at 30 weeks, another gynecological visit is required before 36 weeks, including cervical check, blood pressure measurement, and, if necessary, an ultrasound examination.

Week 36

Laboratory tests, vaginal discharge culture, CTG (cardiotocography) examination, cervical check, ultrasound examination, blood pressure measurement, and urine test.

Following this, until the scheduled date, weekly CTG, blood pressure, urine, gynecological, and ultrasound examinations, including flow measurement.

The above schedule can be influenced by numerous factors, such as the mother's age, previous illnesses, the history of past pregnancies, any conditions developed during the current pregnancy (e.g., high blood pressure, diabetes, threatened preterm labor...), or even something as simple as Christmas or a family vacation. Fortunately, few tests have extremely strict timing that we cannot deviate from; for everything else, we can adjust according to needs, considering both the professional protocol and the highest safety of the expectant mother.
